Venoarterial extracorporeal membrane oxygenation to rescue sepsis-induced cardiogenic shock: A retrospective, multicentre, international cohort study

Bréchot N, Hajage D, Kimmoun A, et al. - This retrospective, multicentre, international cohort study was undertaken to investigate whether venoarterial extracorporeal membrane oxygenation (VA-ECMO) support affords an effective approach to manage patients with sepsis-induced cardiogenic shock refractory to conventional treatments. Comparison was performed between patients (n = 82, aged ≥18 years) with septic shock who were treated with VA-ECMO vs controls (n = 130, not receiving ECMO). To control for confounders, a propensity score-weighted analysis was performed. Findings showed a large and significant improvement in survival of patients with severe sepsis-induced cardiogenic shock managed with VA-ECMO vs controls not receiving ECMO. However, unmeasured confounders could not be ruled out despite the careful propensity-weighted analysis.
