Value of neutrophil to lymphocyte ratio and its trajectory in patients hospitalized with acute heart failure and preserved ejection fraction

Boralkar KA, Kobayashi Y, Amsallem M, et al. - In patients hospitalized with acute heart failure with preserved ejection fraction (HFpEF), researchers investigated if the neutrophil to lymphocyte ratio (NLR) on admission and NLR trajectory would be complementary to the Get with the Guidelines Heart Failure (GWTG-HF) risk score. They selected 443 cases with complete data of NLR both on admission and at discharge, using the Stanford Translational Research Database and manual curation. All-cause mortality was the primary endpoint. The study sample had a mean age of 77 ± 16 years, and 58% were female. Concerning outcome prediction, both NLR on admission and absolute NLR trajectory between admission and discharge were incremental to GWTG-HF risk score, as observed on multivariable Cox modeling. A significantly improved area under the operator-receiver curve and reclassification up to 3 years following admission were achieved by adding NLR or absolute NLR trajectory to the GWTG-HF risk score. Overall, the possible usefulness of NLR for risk stratification in patients hospitalized with HFpEF was suggested.
