Two-year real-world results of lumbar discectomy with bone-anchored annular closure in patients at high risk of reherniation

Ardeshiri A, et al. - Through a prospective, single-center study among 75 high-risk patients with large annular defects of lumbar discectomy for sciatica caused by intervertebral disc herniation with adjunctive annular closure device (ACD) implantation for reduction of herniation recurrence risk, the researchers ascertained the safety and effectiveness of limited lumbar discectomy with additional implantation of an ACD in such patients. A decrease from 73 to 6, 51 to 13 and 49 to 7 in mean leg pain severity, back pain, and Oswestry Disability Index (ODI), respectively, were noticed. Ninety-one percent of patients for leg pain, 65% for back pain, and 94% for ODI attained the minimum important difference. Hence, at a 2-year follow-up, additional implantation of an ACD was reliable with a lower reherniation recurrence rates in patients at high risk of herniation recurrence following limited lumbar discectomy in regular clinical practice, in comparison to reported rates in high-risk patients.
