Tuberculosis diagnosis by metagenomic next-generation sequencing on bronchoalveolar lavage fluid: A cross-sectional analysis

Liu X, Chen Y, Ouyang H, et al. - In view of the established effectiveness of metagenomic next-generation sequencing (mNGS) for diagnosing infectious diseases, researchers sought to demonstrate its potential clinical utility for tuberculosis (TB) diagnosis. They tested 322 bronchoalveolar lavage fluid (BALF) samples obtained from 311 suspected and confirmed pulmonary TB patients, using mNGS, smear, Xpert MTB/RIF (Xpert) and culture. Diagnostic performance of mNGS was compared with conventional detection methods for Mycobacterium tuberculosis complex (MTBC) and other pathogens in BALF. Per findings, combined with mNGS based on conventional detection methods could aid in improving detection rate of MTBC. Vit-D, ESR, TB retreatment and cavity in the chest were identified affecting MTBC detection by mNGS. Prior anti-TB usage within 3 months did not affect MTBC Dx of mNGS. In addition, mNGS could lead to recognition of pathogens in a non-targeted approach for the better diagnosis of coinfection.
