The significance of the small adenoma: A longitudinal study of surveillance colonoscopy in an Australian population

Symonds EL, et al. - The risks of advanced colorectal neoplasia following a baseline finding of a small adenoma compared with advanced adenoma were determined via retrospectively auditing patients enrolled in a colorectal cancer (CRC) surveillance program. The patients were provided with regular colonoscopies and screening with faecal immunochemical test (FIT) in this program. Overall, researchers included 378 patients (62.6±11.2 years, 57.9% male); 44.2% of these were diagnosed with small adenoma and 55.5% with advanced adenoma at baseline. They noted a lower risk of advanced neoplasia following a small adenoma than following an advanced adenoma. However, a positive FIT strongly predicted the risk. For this population, they suggest reducing frequency of colonoscopy while providing regular FIT, as a more efficient way of using resources.
