The impact of pre‐existing peripheral artery disease on transcatheter aortic valve implantation outcomes: A systematic review and meta‐analysis

Ueshima D, Barioli A, Fovino LN, et al. - Researchers undertook a meta‐regression analysis to evaluate the results of patients with and without peripheral arterial disease (PAD) undergoing transcatheter aortic valve implantation (TAVI). They searched for investigations published between January 2002 and March 2018 and describing outcomes based on the presence of PAD in TAVI patients. Short‐, mid‐ and long‐term mortality, and peri‐procedural vascular complication (VC) were the outcome measures that were assessed. This analysis included 26 studies (68,581 TAVI patients, of whom 17,326 with preprocedural PAD). Findings revealed an increased risk of all‐cause mortality and VC post-TAVI in relation to the presence of pre‐existent PAD. Both in PAD and non‐PAD patients, adoption of smaller sheaths during the procedure appeared to be related to fewer peri‐procedural VC.
