The impact of arterial hypertension on left ventricular strain in patients with aortic stenosis and preserved ejection fraction

Tadic M, et al. - In this cross-sectional study, researchers assessed LV strain in patients with moderate and severe aortic stenosis in relation to hypertension. Participants were 115 patients with moderate and severe aortic stenosis and preserved LV ejection fraction (>50%) and 89 age-matched normotensive and hypertensive controls. All the participants had comprehensive echocardiographic examination. Two groups were defined: normotensive and hypertensive groups. Hypertensive patients with severe aortic stenosis had remarkably lower LV global longitudinal strain, as well as endocardial and epicardial longitudinal strains, compared with their normotensive counterparts. In patients with significant aortic stenosis, LV mechanics were additionally significantly affected in a negative way by hypertension. Independently of main clinical and demographic features, relations of blood pressure with LV global longitudinal and circumferential strains were evident in aortic stenosis patients.
