The established electrocardiographic classification of anterior wall myocardial infarction misguides clinicians in terms of infarct location, extent and prognosis

Bozbeyoğlu E, et al. - In view of the recent evidence that the currently used scheme for the classification of infarct location and extent in anterior myocardial infarction (MI) may be misleading both in anatomic and prognostic sense, researchers examined electrocardiograms (ECG) of 379 patients with the diagnosis of anterior MI to first classify them according to established scheme and then reassessed these using newer criteria for angiographic site of occlusion. They used multiple angiographic views to determine the site of left anterior descending (LAD) occlusion. Classification of patients using the established scheme neither provided prognostic information nor assisted in accurate localization of infarction. They recommend regarding it as obsolete and abandoning its use. Instead, newer criteria provided by the angiographic correlation studies may help to assess the extent of infarction.
