TAVR for low-flow, low-gradient aortic stenosis: Prognostic impact of aortic valve calcification

Ludwig S, Goßling A, Waldschmidt L, et al. - Researchers performed this retrospective single-center study to determine the prognostic influence of aortic valve calcification (AVC) among patients with low-flow, low-gradient aortic stenosis (AS) undergoing transcatheter aortic valve replacement (TAVR). This study included all cases managed with TAVR for severe low-flow, low-gradient AS (n = 526), ie, low EF low gradient AS (LEF-LG AS; n = 290) and paradoxical low-flow, low-gradient AS (PLF-LG AS; n = 236); AVC was quantified in all. Patients were trichotomized based on gender-specific AVCdensity (calcium volume per annulus area) tertiles in both subgroups. AVCdensity was shown to be independently related to better survival in LEF-LG AS patients, but not in those with PLF-LG AS, in multivariable analysis. Based on the findings, experts suggested the likely diagnostic as well as prognostic value of AVC quantification because it makes it easy to select LEF-LG AS patients with higher likelihood of beneficial result following TAVR.
