Surgical treatment of type II odontoid fractures in elderly patients: A comparison of anterior odontoid screw fixation and posterior atlantoaxial fusion using the Magerl–Gallie technique

Shousha M, et al. - Between 1994 and 2014, authors studied 133 individuals to determine anterior odontoid screw fixation vs posterior atlantoaxial fusion in the surgical treatment of type II B odontoid fractures in elderly subjects according to Grauer. They noted a shorter mean operative time in group A (cases with age 74.19 years) (64.5 min) as compared to group B (cases with age 78.16 years) (116 min). They also noticed the mean amount of blood loss in group A (79 ml) significantly less than in group B (379 ml). A shorter span of 17.4 days in group A was observed as a mean postoperative hospital stay vs group B ie, 30 days. A higher rate of pseudoarthrosis in group A (25.5%) vs group B (3.5%) was also recorded. Overall, they suggested odontoid screw fixation, a less invasive surgery for type II B odontoid fractures in elderly subjects.
