Single coil implantable cardioverter defibrillator leads in patients with hypertrophic cardiomyopathy

Kumar KR, Mandleywala SN, Madias C, et al. - Given that termination of ventricular fibrillation (VF) in patients with hypertrophic cardiomyopathy (HC) may need higher energies; therefore, dual coil defibrillation leads are frequently implanted, however, single coil leads may be favored in young patients, so, researchers investigated this topic in this study with all patients with HCM implanted with a transvenous ICD from years 2000 to 2014. There were 249 patients, among these, 223 had VF testing including 150 with a dual coil lead and 73 a single coil. Lower successful VF energies were found in patients tested with dual coil vs single coil. In 97.3% of patients, adequate safety margin for defibrillation was reported. Findings revealed the ability of single coil leads in affording adequate safety margins in HC patients undergoing ICD implantation. Considering defibrillation testing in all HC patients receiving ICD implantation was recommended. Defibrillation testing was also advised in those undergoing implantation with a single coil lead.
