Retrospective analysis of surgical outcomes for atlantoaxial subluxation

Yamada T, et al. - In this study, researchers examined the data to estimate the outcomes of various surgical techniques for atlantoaxial subluxation (AAS). They observed that subjects in the C1 laminectomy (LAM) group were older as compared to those in the C1/2 fixation group (74.6 years vs 68.0 years). They noticed the following average recovery rate from the preoperative status: the C1 LAM group, 39.4%; the C1/2 fixation group, 49.8%. An improvement in C-JOA score from 9.8 to 13.1 points was significantly observed after surgery in the C1/2 fixation group. A reduction in the postoperative atlantodental interval (ADI) from 8.6 mm to 3.8 mm in the good outcome group after fixation. Overall, they confirmed the efficacy of C1/2 fixation technique in terms of neurological recovery. Although, a high complication rate in surgeries for AAS, especially in the C1/2 fixation was also noted. They suggested C1 LAM for high-risk AAS cases like in case of elderly subjects with various complications.
