Resolution of symptoms and resumption of sex after diagnosis of nongonococcal urethritis among men who have sex with men

Chambers LC, Hughes JP, Glick SN, et al. - Researchers examined men who have sex with men who received azithromycin (1 g) for nongonococcal urethritis (NGU) at the Public Health–Seattle and King County STD Clinic for the resolution of Symptoms and resumption of sex after diagnosis of nongonococcal urethritis. On web-based diaries, participants reported daily urethral symptoms and sexual activity over 12 weeks of follow-up. Urethral symptoms or visible urethral discharge plus 5 or greater polymorphonuclear leukocytes per high-power field defined NGU. They defined the time of symptom resolution as the first of 5 consecutive asymptomatic days. This study comprised 100 participants with NGU and no Chlamydia trachomatis (CT)/Mycoplasma genitalium (MG) coinfection; of these, 36 (36%) had CT-NGU, 22 (22%) had MG-NGU, and 42 (42%) had non-CT/non-MG NGU. Among men who have sex with men treated with azithromycin for nongonococcal urethritis, symptoms lasted longer than 7 days in 37%, and urethral sexual exposure, as recommended, was not avoided by 27%. These findings emphasize educating patients during counseling at NGU diagnosis that symptoms may persist more than 7 days, particularly for non-CT NGU, and highlighting the rationale for the 7-day abstinence period.
