Relation of clinical presentation of aortic stenosis and survival following transcatheter aortic valve implantation

Ben-Shoshan J, et al. - In this study performed on a consecutive cohort of severe aortic stenosis (AS) patients (n=862, mean Society of Thoracic Surgeons (STS) score 4.16±2.9) who underwent transfemoral transcatheter aortic valve implantation (TAVI) between 2009-2016, researchers examined the correlation between the severity of AS presenting symptoms and survival post-TAVI. Two groups of patients were those having severe symptoms (ie, angina, syncope or heart failure, n=424) or those with mild symptoms (ie, dizziness, fatigue, effort dyspnea, chest discomfort, n=438). These groups did not differ with respect to device success and in-hospital complications. A median follow-up of 2.84 (1.9-4.5) years was performed, during which, the survival at 1, 3 and 5 years in the entire cohort, was 89±1.1%, 75±1.6% and 59±2.1%, respectively. Higher mortality was observed in relation to severe symptoms. Findings revealed an impact of severity of AS symptoms on post-TAVI survival. An independent predictor of early mortality was overt heart failure. The unfavorable impacts of clinical progression on post-TAVI survival can be reduced via implementing early intervention following diagnosis of severe AS.
