Radiofrequency ablation for subcardiac hepatocellular carcinoma: Therapeutic outcomes and risk factors for technical failure

Cha DI, et al. - In this retrospective study, researchers compared the therapeutic results and safety of radiofrequency (RF) ablation for subcardiac [defined as an index tumor that was located ≤ 1 cm from the pericardium in axial or coronal images] and non-subcardiac hepatocellular carcinoma (HCC) and assessed the risk factors for technical failure of the procedure. For this investigation, they involved 73 patients with subcardiac HCC and the same number of patients with non-subcardiac HCC matched by tumor size between September 2002 and May 2016. Using multivariable logistic analysis, prognostic factors for technical failure were assessed. Distance of the index tumor from the heart (cutoff value of 0.5 cm) and the location of the tumor (segment IV) were risk factors for technical failure. RF ablation has been an effective treatment in terms of local tumor progression (LTP) for subcardiac HCC. Overall, the investigators concluded that RF ablation has been an effective subcardiac HCC treatment. In treatment planning, however, tumor location should be considered to prevent technical failure.
