Radial vs femoral artery access for percutaneous coronary artery intervention in patients with acute myocardial infarction and multivessel disease complicated by cardiogenic shock: Subanalysis from the CULPRIT-SHOCK trial

Guedeney P, Thiele H, Kerneis M, et al. - This study was carried out to assess the usage and effect of transradial artery access (TRA) compared with transfemoral artery access (TFA) in patients undergoing percutaneous coronary intervention (PCI) for acute myocardial infarction (MI) complicated by cardiogenic shock (CS). In this post-hoc analysis of the CULPRIT-SHOCK trial, researchers randomized patients presenting with MI and multivessel disease complicated by CS to a strategy of culprit-lesion-only or immediate multivessel PCI. The association between the arterial access and outcomes was evaluated using multivariate logistic models. A total of 673 patients were included in the study. The findings reveal that TRA may be correlated with improved early outcomes, while the reason for this finding requires future study in patients undergoing PCI for acute MI complicated by CS.
