Quantitative IFN-γ release assay and tuberculin skin test results to predict incident tuberculosis: A prospective cohort study

Gupta RK, Lipman M, Jackson C, et al. - Researchers investigated if an improved prediction of incident tuberculosis (TB) could be achieved by implementing higher diagnostic thresholds than currently advised for QuantiFERON Gold-in-Tube (QFT-GIT), T-SPOT.TB, and the tuberculin skin test (TST). They extended the follow-up of a UK cohort including 9,610 adult TB contacts and recent migrants, via relinkage to national TB surveillance records (median follow-up 4.7 yr). Findings revealed that a modest increase in the positive predictive value for incident TB but marked decrease in sensitivity was afforded by the implementation of higher thresholds for QFT-GIT, T-SPOT.TB, and TST. To enhance incident TB prediction, new biomarkers or validated multivariable risk algorithms are needed.
