QRS vector magnitude as predictor of ventricular arrhythmia in patients with Brugada syndrome

Ragab AAY, et al. - Using a retrospective cohort with 136 patients (47±15 years, 66% male), researchers assessed the value of the QRS vector magnitude (QRSvm) in predicting ventricular tachyarrhythmia (VTA) in patients with Brugada syndrome (BrS). They analyzed all medical records and used Kors' quasi-orthogonal method to reconstruct all 12- lead ECGs into QRSvm. They looked for the presence of electrocardiographic signs suggestive of right ventricular outflow tract (RVOT) conduction delay, including R wave sign, deep SI, SII>SIII pattern, and Tzou criteria. For QRSvm, the area under receiver operator characteristic curve was 0.85. Sensitivity and specificity of 89% and 71%, respectively, were yielded by QRSvm cutoff of 1.55mV. Overall, VTA events in BrS patients could be predicted by electrocardiographic signs indicative of RVOT conduction delay as well as by QRSvm.
