Propensity-matched comparison of cryoballoon and radiofrequency ablation for atrial fibrillation in elderly patients

Ikenouchi T, et al. - In this single-center retrospective study of 305 patients older than 75 years with paroxysmal and persistent atrial fibrillation (AF) who had pulmonary vein isolation (PVI) between January 2012 and August 2017, researchers assessed the clinical outcomes of cryoballoon (CB) ablation vs radiofrequency (RF) ablation. Using propensity scores, patients were matched and were assessed for AF/atrial tachycardia recurrence at 12-month follow-up. This study included 198 patients (99 matched pairs), with mean age 78 ± 2 years and ratio of paroxysmal AF being 83%. In the CB group, a significantly lower mean procedure time was reported. PVI with a CB or RF yielded no significantly differ clinical outcomes in elderly patients with non–pulmonary vein foci that were all successfully ablated, as revealed on analyzing Kaplan-Meier estimates. Based on the findings, they concluded that PVI with a CB might offer an efficacy that is comparable to that of PVI with RF in AF patients older than 75 years.
