Progression of ECG abnormalities associated with initial ventricular fibrillation in asymptomatic patients with Brugada syndrome

Morita H, et al. - In this study involving patients with ventricular fibrillation (VF) and consecutive asymptomatic patients with Brugada syndrome (BrS), researchers intended to define the electrophysiological change that signal VF in BrS. For this purpose, they assessed the changes of ECG at the beginning of VF vs ECGs recorded when patients were still asymptomatic. They assessed ECGs in asymptomatic patients at two time points with more than 6 months in between. Various ECG markers including type 1 ECG, fragmented QRS (fQRS, multiple spikes within the QRS complex) were also evaluated. In association with the initial VF onset, they noted the development of QRS and ST-T abnormalities. They observed VF was promoted by aggravation of the conduction disturbance over and above BrS-ECG
