Prognostic value of impaired hepato-renal function assessed by the MELD-XI score in patients undergoing percutaneous mitral valve repair

Spieker M, et al. – Researchers investigated the prognostic utility of the Model for End-Stage Liver Disease (MELD)-XI score—originally developed for prognostic assessment in patients with advanced liver disease—in patients undergoing percutaneous mitral valve repair (PMVR) with the MitraClip system. Researchers calculated MELD-XI score for 192 consecutive patients undergoing PMVR on the day of the procedure. Patients were grouped based on MELD-XI score and compared with regard to clinical characteristics and outcomes following PMVR. Findings demonstrated high mortality in patients with a MELD-XI score > 16, indicating the importance of a high-risk preoperative assessment for these patients. Given the considerable 1-year mortality post-PMVR, the authors recommended careful discussion within the heart team for the best treatment option for these patients.
