Prognostic implications of right ventricular free wall longitudinal strain in patients with significant functional tricuspid regurgitation

Circulation: Cardiovascular Imaging Mar 22, 2019

Prihadi EA, et al. - In patients with substantial functional tricuspid regurgitation, researchers assessed the prevalence and prognostic ability of impaired right ventricular (RV) free wall longitudinal strain vs tricuspid annular plane systolic excursion (TAPSE) and fractional area change (FAC). Overall, 896 patients (51.3% men, 71 years [62–78 years]) were included. TAPSE <17 mm, FAC <35%, and RV free wall longitudinal strain >−23% defined RV dysfunction and participants were divided based on its presence. Follow-up was performed to determine the occurrence of all-cause mortality. Higher rates of RV dysfunction were detected by impaired RV free wall longitudinal strain in this study population. Beyond conventional echocardiographic parameters of RV systolic function, an association of impaired RV free wall longitudinal strain with worse outcome was evident.
