Prognostic impact of heart failure history in patients with secondary mitral regurgitation treated by MitraClip

Adamo M, Gavazzoni M, Castiello A, et al. - This retrospective analysis was performed to determine the impact of heart failure (HF) history on the prognosis of patients with secondary mitral regurgitation (SMR) undergoing MitraClip. This study involved 186 patients with SMR receiving MitraClip at 4 centres. Number or days of HF hospitalizations in the 12-months prior to MitraClip, or time from last HF hospitalization to MitraClip, or time between first SMR diagnosis and MitraClip, all defined HF history. An increased risk of clinical events was observed in relation to only the number of HF hospitalizations before MitraClip (among variables associated with HF history), while prognosis remained unaffected by days of prior HF hospitalization, time from last HF hospitalization and from first diagnosis of SMR. Overall, findings revealed that the most powerful predictor of prognosis among variables associated with HF history was recurrence (>1) of HF hospitalizations prior to MitraClip. The outcomes were not impacted by latency of intervention.
