Prognostic impact of atrial rhythm and dimension in patients with structural heart disease undergoing cardiac sympathetic denervation for ventricular arrhythmias

Dusi V, Sorg JM, Gornbein J, et al. - Researchers examined how atrial arrhythmias (AAs) and left atrial volume index (LAVI) influence cardiac sympathetic denervation (CSD) outcome in patients with structural heart disease (SHD). Further, they assessed changes in AA burden and in atrial pacing following CSD. They analyzed 91 patients (mean age 56 ± 13 years; mean left ventricular ejection fraction 34% ± 14%; 47% with a history of AAs) who underwent left CSD (16%) or bilateral CSD (BCSD) from 2009 to 2018. LAVI was identified as predictive of death/orthotopic heart transplant in patients with SHD undergoing CSD. Following BCSD, there was no change in AA burden, which was already low at baseline, while there was an increase in the need for atrial pacing, implying an impact of BCSD on sinus node chronotropism.
