Outcomes of urgent vs nonurgent transcatheter aortic valve replacement

Elbadawi A, Elgendy IY, Mentias A, et al. - Researchers investigated the consequences of transcatheter valve replacement (TAVR) done in an urgent clinical setting. They selected hospitalizations for TAVR in the urgent setting [n = 10,114 (24%)] from the Nationwide Inpatient Sample database years 2011–2014. They performed a comparison between patients who had TAVR in nonurgent vs urgent settings, using propensity score matching. Among all TAVR procedures performed, approximately a quarter was performed in the urgent settings in this large nationwide study. Higher mortality and increased complications were reported in relation to urgent TAVR vs nonurgent TAVR, but there was no remarkably higher absolute difference in in-hospital mortality, and therefore, TAVR can be viewed as a sensible approach when indicated.
