Outcomes of His-bundle pacing upgrade after long-term right ventricular pacing and/or pacing-induced cardiomyopathy: Insights into disease progression

Vijayaraman P, et al. - Researchers examined patients with chronic right ventricular pacing (RVP) and longstanding atrioventricular block (AVB) to retrospectively evaluate the feasibility of His-bundle pacing (HBP) in these subjects. They also focused on the effectiveness of HBP in reversing RVP-induced adverse remodeling. Among participants, they performed electrocardiographic and echocardiographic evaluations at baseline and follow-up. Findings revealed that HBP led to normalization of QRS complexes and T waves with stable thresholds, although the duration of AVB and chronic RVP was long. This finding is indicative of an uncommon occurrence of progression of distal conduction disease in this population. HBP consistently reversed electrical and structural alterations induced by chronic RVP.
