Outcomes in Kawasaki disease patients with coronary artery abnormalities at admission

Ae R, Abrams JY, Maddox RA, et al. - To acquire knowledge about sub-acute outcomes of coronary artery lesions (CALs) identified at admission during Kawasaki disease (KD), researchers performed this nationwide Japanese KD survey. This analysis included KD patients with CALs identified at admission and who were administered standard IVIG treatment within 10 days of disease initiation. Experts assessed coronary outcomes through alterations in CALs from admission to the first evaluation at 30 days from disease initiation in three groups: improved, unchanged, and progressed. Among 2,024 patients with CALs identified at admission, 1548 (76.5%), 390 (19.3%), and 86 (4.2%) patients were found to have improved, unchanged, and progressed outcomes, respectively. Improved outcome was noted in over 80% of patients with coronary artery dilatations. It was inferred that more rigorous treatment may be needed for KD patients with larger CALs, ≥ 60 months old, and with recurrent status or parental history. Additionally, worse coronary outcome may be seen in relation to delayed admission, suggesting the need for prompt diagnosis and treatment.
