Outcomes and safety of same-day discharge after percutaneous coronary intervention: A 10-year single-center study
Catheterization and Cardiovascular Interventions Feb 04, 2019
Rubimbura V, et al. - While randomized and observational studies suggest same-day discharge (SDD) following percutaneous coronary intervention (PCI) is safe and cost-effective, concerns about early adverse events have led to limited acceptance, so researchers assessed 1,635 unselected patients who had PCI for early outcomes after SDD PCI, primarily stable ischemic heart disease (SIHD). Of the 1,635 unselected patients, 1,073 (65.6%) were discharged on the same day as their procedure, most of whom underwent ad hoc PCI while 562 (34.4%) were admitted due to adverse events during PCI (n = 60), or within the next 4–6 hours (n = 52) or mainly due to physician preference (n = 450). In most patients with SIHD, SDD following successful PCI with no complications within the next 4–6 hours was safe and feasible. Only two readmission and no major cardiac adverse events such as death or stent thrombosis were noted among 1,035 SDD patients treated over 10 years. These findings support the safety of SDD for the patient and its cost-efficacy for the healthcare system, and its more widespread implementation is recommended.
