Outcome of patients undergoing transcatheter implantation of aortic valve with previous mitral valve prosthesis (OPTIMAL Study)

Baldetti L, et al. - In a retrospective registry, researchers assessed procedural and early outcomes of 154 patients with previous mitral valve (MV) prosthesis who underwent transcatheter aortic valve replacement (TAVR). These patients had TAVR, at a mean of 11.7±8.4 years after mitral surgery, across high-volume medical Centers. In 150 (97.4%) patients, procedural success was reported, with attenuation of aortic gradients. In 133 (86.3%) cases, device success was achieved. Peri-procedurally, the following complications were reported: 4 (2.6%) cerebrovascular accidents; 10 (6.6%) major vascular complications; 22 (14.4%) severe bleedings; 1 (0.7%) myocardial infarction; and 5 (3.2%) in-hospital deaths. Findings suggested the safety as well as feasibility of TAVR in patients with a previous mitral prosthesis. TAVR offered good hemodynamic outcomes at 30-day and at longer term follow-up in these patients.
