Outcome of normal-flow low-gradient severe aortic stenosis with preserved left ventricular ejection fraction: A propensity-matched study

Chadha G, et al. - Researchers assessed the outcome of patients with normal-flow, low-gradient severe aortic stenosis (NF-LG-SAS)—defined by aortic valve area < 1 cm2, mean gradient < 40 mm Hg, and indexed stroke volume > 35 mL/m2—and compared those with the outcome in moderate aortic stenosis (AS) [MAS] and with high-gradient severe-AS (HG-SAS). Overall 154 patients with NF-LG-SAS were included. Patients with MAS (aortic valve area between 1.0 and 1.3 cm2) were 366 and those with HG-SAS were 1,055 in total. Findings revealed that the outcome observed in patients with NF-LG-SAS was comparable to those with MAS provided a guideline-directed aortic valve replacement was performed during follow-up, mostly at the stage of HG-SAS. In order to recognize progression to true severe AS in NF-LG-SAS, the significance of rigorous echocardiographic evaluation to rule out measurement errors and close follow-up were emphasized.
