Optimal threshold of left ventricular ejection fraction for aortic valve replacement in asymptomatic severe aortic stenosis: A systematic review and meta‐analysis

Perry AS, et al. - Researchers examined the link between left ventricular ejection fraction (LVEF) and mortality advantage in comparing early aortic valve replacement (AVR) vs watchful waiting in asymptomatic patients having severe aortic stenosis (AS). Observational studies as well as randomized controlled trials including adults with asymptomatic severe AS were identified from MEDLINE, Embase, Web of Science, and Google Scholar. Inclusion of studies comparing AVR with conservative management was done. Experts found 10 eligible studies comprising 3332 patients. Findings revealed a mortality benefit, across the spectrum of LVEF, in relation to AVR in asymptomatic patients with high‐gradient severe AS. The data generated herein call into question the requirement of an LVEF threshold for advising AVR in this patient group.
