Operative treatment vs nonoperative treatment of Achilles tendon ruptures: Systematic review and meta-analysis

Ochen Y, et al. - In this systematic review and meta-analysis, researchers compared re-rupture rate, complication rate, and functional outcome after operative vs nonoperative treatment of Achilles tendon ruptures and compared re-rupture rate after early and late full weight bearing, and evaluated re-rupture rate after functional rehabilitation with early range of motion, as well as compared effect estimates from randomised controlled trials and observational studies. Compared to nonoperative treatment, operative treatment of Achilles tendon ruptures decreases the risk of re-rupture. However, re-rupture rates were low and differences between treatment groups were small, with a risk difference of 1.6%. Operative treatment leads to a greater risk of other complications (risk difference 3.3%). The final decision to manage acute Achilles tendon ruptures should be based on patient-specific factors and shared decision-making. Infection was the main complication after operative treatment which occurred in 2.8% of patients.
