Natriuretic peptide response and outcomes in chronic heart failure with reduced ejection fraction

Januzzi Jr. JL, Ahmad T, Mulder H, et al. - A total of 638 study individuals who were alive and had available amino-terminal pro–B-type natriuretic peptide (NT-proBNP) results, 90 days following randomization were involved in this study by the researchers in order to investigate the prognostic meaning of NT-proBNP changes after heart failure (HF) therapy intensification with regards to the goal NT-proBNP value of 1,000 pg/ml examined in the GUIDE-IT trial. At 90 days, a total of 198 individuals had an NT-proBNP ≤ 1,000 pg/ml with no variation in the attainment of NT-proBNP goal between the biomarker-guided and usual care arms. From cardiovascular (CV)/HF hospitalization or all-cause mortality and lower adjusted hazard of subsequent HF hospitalization/CV death and all-cause mortality, NT-proBNP ≤ 1,000 pg/ml by 90 days was related to longer freedom and irrespective of increased baseline concentration, an NT-proBNP ≤ 1,000 pg/ml at 90 days was correlated with better outcomes and significantly better Kansas City Cardiomyopathy Questionnaire overall scores. Thus, patients with heart failure with decreased ejection fraction whose NT-proBNP levels decreased to ≤ 1,000 pg/ml during guideline-directed medical therapy had better outcomes. These conclusions may aid in understanding the outcomes of the GUIDE-IT trial.
