Nationwide outcomes of aortic valve replacement for pure aortic regurgitation in Germany 2008–2015

Stachon P, et al. - In this analysis of 138,237 cases of aortic valve replacement in Germany, researchers analyzed the utilization of surgical aortic valve replacement (SAVR) and transcatheter aortic valve replacement (TAVR) for aortic regurgitation (AR), on a nationwide level, from 2008 until 2015. Of overall SAVR-cases and TAVR cases, 13.2% and 1.3%, respectively, were performed in AR. Young age and lower logistic EuroSCORE characterized AR patients undergoing SAVR. Nevertheless, higher stroke rates, bleedings, prolonged mechanical ventilation, and in-hospital mortality were evident. A higher operative risk was noted among patients with AR in the TAVR group. Post-TF(transfemoral)-TAVR in AR vs stenosis, lower stroke rates but more frequent occurrence of bleedings and prolonged ventilation was noted. A marked variation was noted in the mortality (TF-TAVR: 15.2% in 2011; 2.8% in 2015; transapical-TAVR: 17.7% in 2012 and 0% in 2014). In clinical practice, TAVR is off-label used in AR. As far as in-hospital outcomes are concerned, TAVR appears to be a safe choice for AR.
