Myocardial inflammation and dysfunction in COVID-19–associated myocardial injury

Weckbach LT, Curta A, Bieber S, et al. - Presence of myocardial injury, defined by elevated troponin levels, in patients with coronavirus disease 2019 (COVID-19) is associated with adverse outcome among them. Researchers here prospectively assessed 18 patients with COVID-19 and myocardial injury using transthoracic echocardiography, cardiac magnetic resonance imaging, and endomyocardial biopsy. In echocardiography, normal to mildly decreased left ventricular ejection fraction of 52.5% was noted but with moderately to severely decreased left ventricular global longitudinal strain of −11.2% (−7.6% to −15.1%). The presence of myocardial injury in these patients was evidenced by decreased echocardiographic left ventricular strain, myocarditis patterns on cardiac magnetic resonance, and increased macrophage numbers but with no predominance of lymphocytic myocarditis in endomyocardial biopsies.
