Mortality and self-harm in association with clozapine in treatment-resistant schizophrenia
American Journal of Psychiatry Oct 16, 2017
Wimberley T, et al. - In individuals with treatment-resistant schizophrenia, the authors aimed to assess rates of all-cause mortality and self-harm in association with clozapine treatment. Among individuals with treatment-resistant schizophrenia not treated with clozapine, the outcomes showed a nearly twofold higher mortality rate compared with clozapine-treated individuals. The results also proposed a harmful effect of other antipsychotics regarding self-harm compared with clozapine. It remained to be examined to what extent the observed excess mortality after clozapine discontinuation was confounded by nonadherence and other unobserved factors and to what extent it was mediated by adverse effects from recent clozapine exposure or deterioration in physical or mental health precipitated by clozapine discontinuation.
