Monitoring of older HIV-1-positive adults by HIV clinics in the United Kingdom: A national quality improvement initiative

Ekong N, Curtis H, Ong E, et al. - Researchers sought to delineate a UK-wide process to evaluate adherence to guidelines for the routine investigation and monitoring of HIV-positive adults aged ≥ 50 years and present clinical services with individual feedback to promote improvement in the quality of care. The British HIV Association (BHIVA) invited HIV clinical care sites to provide retrospective data from case notes of up to 40 adults aged ≥ 50 years with HIV-1 infection attending the clinic for care during 2017 and/or 2018, using a structured dynamic online questionnaire. HIV clinical care sites were invited by the BHIVA to present retrospective data from case notes of up to 40 adults aged ≥ 50 years with HIV-1 infection visiting the clinic for care during 2017 and/or 2018, using a structured dynamic online questionnaire. They received 4,959 questionnaires in return from 141 clinical services. The analysis revealed meeting up of targets for viral load and blood pressure monitoring nationally but not for cardiovascular disease risk assessment, smoking status documentation and recording of comedication. The variable performance was observed concerning other outcomes; adherence and laboratory measurements were assessed more frequently than lifestyle and wellbeing assessments.
