Microvascular abnormalities and long-term efficacy after stereotactic radiotherapy under continued intravitreal anti-VEGF treatment for neovascular AMD

Hatz K, Zimmermann F, Lazaridis E, et al. - In the present study, the researchers sought to assess the long-term effectiveness and safety of adjunct stereotactic radiotherapy (SRT) to anti-vascular endothelial growth factors (VEGF) injections in a treat-and-extend regimen in neovascular age-related macular degeneration (nAMD). Three years after single-session SRT, a safety analysis including standardized study imaging, and a reading centre based image analysis were followed by 50 consecutive patients after 2 years. Low-energy SRT in real life was associated with a decreased frequency of anti-VEGF injection over year 3. However, due to the reported loss in visual acuity and the extraordinary number of microvascular abnormalities, close follow-up is recommended for these patients. Real-life use, optimal treatment schedule and dose should be urgently addressed again.
