Meta-analysis of transcatheter aortic valve replacement in low-risk patients

Kheiri B, Osman M, Bakhit A, et al. - An electronic database search was conducted for five RCTs ( n = 3,072 patients) by the researchers in order to assess the efficiency and safety of transcatheter aortic valve replacement (TAVR) in comparison with surgical aortic valve replacement (SAVR) in low surgical risk patients. In contrast with SAVR, TAVR was related to a significantly decreased rate of death or disabling stroke, atrial-fibrillation, and postprocedure bleeding, along with a significantly enhanced quality-of-life at 1-year. Nonetheless, TAVR was related to significantly elevated rates of paravalvular leak and pacemaker insertion in comparison with SAVR. In conclusion, TAVR vs SAVR is correlated with a lower rate of death or disabling stroke, among older low-risk patients with severe aortic stenosis. Transcatheter aortic valve replacement is also correlated with improved quality-of-life, decreased bleeding and atrial fibrillation, but greater paravalvular leak and pacemaker implantation rates.
