Meta-analysis of optimal timing of coronary intervention in non-ST-elevation acute coronary syndrome

Barbarawi M, Kheiri B, Zayed Y, et al. - Since coronary angiography is suggested for patients with non-ST-elevation acute coronary syndrome (NSTE-ACS), however, the optimal timing for this remains doubtful, researchers contrasted the safety and effectiveness of early vs delayed invasive management of NSTE-ACS. By two independent authors, literature search of Pubmed/MEDLINE, Cochrane Library, and Embase for all RCTs that contrasted early with delayed invasive approaches in treating NSTE-ACS was performed. Fourteen randomized controlled trials (9,637 patients, mean age 65.4) were included. Among individuals with NSTE-ACS, an early invasive strategy was linked to a lower incidence of MACE and recurrent ischemia vs delayed invasive strategy. No significant differences were found in all-cause mortality, cardiovascular mortality, myocardial infarction, or bleeding events between groups.
