Left ventricular global longitudinal strain is associated with long-term outcomes in moderate aortic stenosis

Zhu D, Ito S, Miranda WR, et al. - Given a link of left ventricular global longitudinal strain (GLS) with long-term results of patients with severe aortic stenosis, researchers focused on its prognostic worth in patients experiencing moderate aortic stenosis. They identified 287 patients (median age 76 years; 47% male) who had received a diagnosis of moderate aortic stenosis (1.0 < aortic valve area ≤1.5 cm2) and had left ventricular ejection fraction ≥ 50%. By means of 2-dimensional strain imaging using speckle-tracking method, the evaluation of GLS was done. They evaluated all-cause death based on the median GLS value. At 1, 3, 5 years, mortality rates were estimated to be 21%, 35%, 48%, respectively, in patients with GLS >−15.2%, and these values were 6%, 15%, 19% in those with GLS ≤−15.2%. Higher mortality rates were observed in relation to impaired GLS in moderate aortic stenosis patients, this was noted even among those undergoing aortic valve replacement.
