Late electrocardiographic changes in patients with new-onset left bundle branch block following transcatheter aortic valve implantation
The American Journal of Cardiology Dec 18, 2019
Faroux L, Muntané-Carol G, Urena M, et al. - Researchers performed this multicenter study on patients who developed new-onset persistent left bundle branch block (NOP-LBBB) following transcatheter aortic valve implantation (TAVI), focusing on the incidence as well as factors related to LBBB recovery and permanent pacemaker implantation (PPI) at 1-year follow-up. Participants were 153 in total (mean age: 81 ± 5 years, 56% of women) having NOP-LBBB after TAVI (balloon-expandable valve in 112 patients). The participants had a clinical visit and 12-lead ECG at 1-year follow-up. They noted recovery of LBBB in 50 patients (33%) and 14 (9%) suffered advanced conduction disturbances necessitating PPI during the follow-up period. At 1-year follow-up, the resolve of NOP-LBBB post-TAVI was observed in one-third of patients. No clinical or ECG variables were found to be related to LBBB recovery. Conversely, an increased risk of PPI within the year post-TAVI was observed in relation to a non-sinus rhythm at baseline and a longer delta PR.
