Is routine urinary screening indicated prior to elective total joint arthroplasty? - A systematic review and meta-analysis

Sousa R, et al. - Via this systematic review and meta-analysis, researchers examined the risk for prosthetic joint infection (PJI) in the presence of asymptomatic bacteriuria (ASB). In addition, they determined the efficacy of preoperative antibiotic treatment in reducing this risk. Screening 663 papers, they included 10 papers (28,588 patients). As per results, ASB patients display an increased risk of developing PJI. However, no relation was evident between most PJI microorganisms and those previously found in the urine. PJI risk remained stable irrespective of preoperative antibiotic therapy. These findings do not support performing routine urinary screening prior to elective total joint arthroplasty and treatment of asymptomatic patients.
