Incidence and short‐term outcomes of surgical bailout after transcatheter mitral valve repair with the MitraClip system

Isogai T, Saad AM, Shekhar S, et al. - Researchers sought to determine the incidence as well as results of surgical bailout (SB) following transcatheter mitral valve repair (TMVr) with MitraClip. They used the Nationwide Readmissions Database 2014–2017 to retrospectively select patients who received TMVr. They defined SB as open thoracotomy for heart and aorta during the same hospitalization. This analysis involved 15,032 eligible patients in total, 214 (1.42%) needed SB, of whom 134 (62.6%) had mitral valve surgery (113 replacements; 21 repairs). Annual hospital volume demonstrated a significant nonlinear, inverse link with SB incidence. In patients with vs without SB, significantly more frequent were in‐hospital death and other in‐hospital adverse events, whereas a similar 30‐day readmission rate was noted. Although a declining incidence of SB post-TMVr was revealed in this study, SB was needed more often in lower‐volume hospitals and was associated with high in‐hospital death. There is a requirement for further efforts to know about the causes for SB as well as to improve results in patients requiring SB.
