Incidence and clinical outcomes of bleeding complications and acute limb ischemia in STEMI and cardiogenic shock

Pahuja M, Ranka S, Chehab O, et al. - In this retrospective cohort study, researchers assessed 10‐year national trends, resource use as well as outcomes of bleeding complications, and acute limb ischemia (ALI) in cardiogenic shock due to ST‐segment elevation myocardial infarction [CS‐STEMI]. A large U.S. national database (National Inpatient Sample) was used to select CS‐STEMI patients. Of 172,491 admissions with CS‐STEMI, bleeding complications and ALI were noted in 31,389 (18.2%) and 1,628 (0.9%), respectively. A rise in overall trends for ALI was seen between 2005 and 2014; however, the number of bleeding events reduced. In the contemporary era, bleeding and ALI are acknowledged as common complications linked with CS‐STEMI. Increased hospital expenses and length of stay were observed in relation to both complications. These data emphasize the requirement to construct algorithms focused on vascular safety in CS‐STEMI.
