Implementation of and early outcomes from anal cancer screening at a community-engaged healthcare facility providing care to Nigerian men who have sex with men

Nowak RG, Ndembi N, Dauda W, et al. - Researchers examined the utilization of high-resolution anoscopy (HRA) as a first-line screening tool for anal cancer in men who have sex with men (MSM) in Abuja, Nigeria. They also assessed the resulting early outcomes. A total of 424 MSM were enrolled between August 2016 to August 2017 in an anal cancer screening sub-study of TRUST/RV368, a combined HIV prevention and treatment cohort. By assessing the identification of squamous intraepithelial lesions (SIL) over time and the proportion biopsied, they evaluated HRA proficiency. With experience over time, HRA proficiency increased. However, low high-grade SIL detection rates were documented, which may have been influenced by obstructed views from internal warts and low biopsy rates; this emphasized the necessity of ongoing assessment and mentoring to confirm this finding. Overall, findings revealed the feasibility of HRA as a first-line screening instrument at an MSM-friendly healthcare facility. Screening could be prioritized by years since anal coital debut and external warts.
