Impact of reperfusion for nonagenarians treated by mechanical thrombectomy: Insights from the ETIS registry

Drouard-de Rousiers E, et al. - Among the 124 nonagenarians, researchers conducted this retrospective analysis to evaluate the effect of endovascular therapy (ET)-related reperfusion on the functional outcome of reperfusion. Between October 2013 and April 2018, they obtained clinical and imaging data from all patients aged over 90 included in the ETIS (Endovascular Treatment in Ischemic Stroke) registry. The connection between post-ET reperfusion and favorable (modified Rankin Scale [0–2] or equal to prestroke value) and good (modified Rankin Scale [0–3] or equal to prestroke value) outcome has been assessed. Findings revealed that those with successful reperfusion had the lowest 90-day modified Rankin Scale among the nonagenarians treated with ET. After the first pass, only patients with successful reperfusion had decreased 90-day mortality and a higher rate of good outcome. Among patients successfully reperfused, no increase in the rate of intracranial hemorrhage was noted. Overall, the authors concluded that successful reperfusion enhances the functional result of nonagenarians who should not be excluded from ET. The first-pass effect should be taken into consideration in this frail population's procedural management.
