Hybrid fixation for total hip arthroplasty showed improved survival over cemented and uncemented fixation: A single centre survival analysis of 2,156 hips at 12-18 years

Journal of Arthroplasty Jun 28, 2019

Fowler AK, et al. - Between 1999 and 2000, 2,156 hips (1,315 cemented, 324 uncemented, and 517 hybrids) that were performed in a single center with follow-up through to 2017 were recruited by the researchers in order to compare the long term survivorship of cemented, hybrid and uncemented total hip arthroplasty (THA) at varying ages. The cumulative incidence of all-cause revision was 10.9% for cemented, 8.9% for uncemented and 6.5% for hybrid, at 18 years. In comparison to hybrid, a statistically significantly higher risk of all-cause revision of cemented fixation in the adjusted model for all ages to 65 years was recognized with a greater risk of revision for loosening, wear or osteolysis at all ages. No advantage over hybrid fixation of uncemented fixation at any age was exhibited, however, benefits over cemented at younger ages for all-cause revision were presented. Therefore, an advantage over cemented hips at all ages was portrayed by hybrid fixation with conventional polyethylene. Further, over cemented only fixation, uncemented THA presented with improved survival at younger ages but no advantage over hybrid THA could be observed.
