Host-response-based gene signatures for tuberculosis diagnosis: A systematic comparison of 16 signatures

Warsinske H, et al. - In this investigation, researchers compared published gene signatures for the diagnosis of active tuberculosis (ATB) across a wide range of publicly available datasets of gene expression, and assessed their performance against World Health Organization (WHO)/Foundation for Innovative New Diagnostics (FIND) target product profiles (TPPs). The investigators identified and compared 16 gene signatures for ATB diagnosis with other clinical conditions. They found that two of the proposed gene signatures met WHO criteria for a triage test based on non-sputum across heterogeneous culture-confirmed datasets. Importantly, in its diagnostic accuracy, only one of these two signatures had a low false-positive rate and no heterogeneity, suggesting that it is generalizable across diverse populations of patients. Outcomes strongly suggested that host-response–based diagnosis of ATB can help achieve the WHO goal of ending TB by 2035 and should be considered for clinical implementation.
