Histopathological diagnosis of prosthetic joint infection: Does a threshold of 23 neutrophils do better than the classification of the periprosthetic membrane in a prospective multicenter study?

Bémer P, et al. - In a multicenter prospective study, researchers assessed whether a threshold of 23 neutrophils does better than the classification of the periprosthetic membrane for histopathological diagnosis of a prosthetic-joint-infection (PJI). Prospectively inclusion of all suspicions of PJI (n=264) was performed between December 2010 and March 2012 in seven centers. For the Morawietz classification, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 78.7%, 90.0%, 96.7%, 52.9% and 81.1%, respectively, and for the 23 neutrophils (NG) threshold, the corresponding values were 82.0%, 90.0%, 96.9%, 57.1% and 83.7%, respectively. Thereby favoring the new algorithm using a threshold of 23 NG to be a new gold standard for the histopathological diagnosis of PJI.
